Certificate Request Form

To request a certificate of Baptism, First Communion, Confirmation or Marriage, Complete the form below.
Please note the following:

You may only request your own certificate, or for your child under 18.
All Certificates must be picked up at the church office in person, with valid ID. We cannot mail, email or fax certificates to you personally.
Out of town request will be mailed to your local Catholic parish, where you can pick them up in person with valid ID. Please provide the Parish name, address and phone number for delivery.
Please allow 4 weeks for St. Joseph to process your request.
